A project summary form is on the Proposal Forms page. The Project Summary Form is a FILLABLE PDF that you can download from the forms page. You may save your work at any time.
In addition to help located within each text field of the PDF (hold your mouse on the field and contextual help will appear). The following instructions should help you to accurately complete the form. Please follow them carefully—the project summary is the most widely consulted description of your project.
The project summary presents a concise description of the funded activity in a form useful to a variety of readers not necessarily requiring detailed information.
TITLE: Project titles should be carefully constructed to give as much information about the project as possible about 16 words but in not more than 150 characters (spaces included). Titles greater than 150 characters will be truncated.
PROJECT NUMBER: Enter your preliminary proposal number (located on your comment sheet included with your acceptance letter). An example of a preliminary proposal number is 000-A-N.
For RENEWALS enter your Project Number, assigned after your proposal was funded (e.g. R/F-187).
PROJECT STATUS: Please enter 1=new.
SUBPROGRAM: From the drop down menu, select the appropriate category for your research project.
REVISION DATE: Please leave this field blank.
INITIATION DATE: Insert the date on which Sea Grant support was/is to start. (02/01/2009)
COMPLETION DATE: Insert the date on which it is estimated that the project will end.
PRINCIPAL INVESTIGATOR(S): Insert the name of the project leader. The last name should be entered first. Please include the full first name and middle initial, e.g., Smith, Jane B.
EFFORT: Please provide the number of months per year the principal investigator spends on the project.
AFFILIATION: Insert the academic affiliation of the project leader (e.g., CSU Long Beach, Electrical Engineering; UC Irvine, Biology).
AFFILIATION CODE: Enter the appropriate affiliation code for the principal investigator's institution from the list provided on page 3 of the fillable PDF Project Summary Form.
CO-PRINCIPAL INVESTIGATOR(S): Insert the names of co-principal investigators. The last name should be entered first. Please include the full first name and middle initial, e.g., Jones, Michael J.
EFFORT: Please provide the number of months per year the Co-Investigator(s) spend on the project.
AFFILIATION:Enter the approrpiate affiliation code for the co-principal investigator, (e.g. CSU Long Beach, Electrical Engineering; UC Irvine, Biology).
AFFILIATION CODE: Enter appropriate affiliation code for the co-principal investigator's institution from the list provided on page 3 of the fillable PDF Project summary form
ASSOCIATE INVESTIGATOR(S): Insert the name of the associate investigator(s). For definitions of personnel categories, see the "Preparing Budget Forms" section. Follow the same rules for entering the names as were used
EFFORT: Please provide the number of months per year the principal investigator/s spends on the project.
AFFILIATION: Insert the academic affiliation of the project leader (e.g., CSU Long Beach, Electrical Engineering; UC Irvine, Biology).

SEA GRANT FUNDS: Enter the total amount of Sea Grant Funds (do not include the trainee stipend) you are requesting for the 2009-2010 grant year. This number should correspond to line "J" under "Extramural Funds" on your Budget Summary Form.
STATE MATCHNG FUNDS: Enter the total amount of Matching Funds you are requesting for the 2009-2010 grant year. This number should correspond to line "L" under "Grantee Share" on your Budget Summary Form.
LAST YEAR SEA GRANT FUNDS: Please leave blank.
LAST YEAR MATCHING FUNDS:Please leave blank.
PASS-THROUGH FUNDS:Please leave blank.

RELATED PROJECTS: Insert the project number(s) of other California Sea Grant projects, please list the project number(s) here.
PARENT PROJECTS: Insert the project number(s) of other California Sea Grant project(s) begun (or completed) earlier, from which this project was derived. The intent is to build a record of continuity.
KEYWORDS: A standard list of keywords from the National Office is in Appendix A (PDF file). Keywords do not need to be restricted to this standard list. You may prefer to use your own word(s), if you decide that another choice is more descriptive or suitable for your purpose. Please list keywords in alphabetical order.
OBJECTIVES: This section should state concisely what the project leader will undertake. Preferably the objectives should be stated so that it can be later determined whether or not they have been accomplished.
METHODOLOGY:This section should contain a brief description of the methods.
RATIONALE: This section should explain in a concise way why the proposed research is appropriate for Sea Grant support.
OUTREACH, COLLABORATION, AND EDUCATION: This section should contain concise statements indicating any significant effect that a research project or outreach activity has on business or industry development, marine resources management, the behavior of user groups, and the advancement of scientific knowledge. In addition, list publications; and names, majors, collaboration, outreach, education, titles of theses, and degrees earned of students supported.
